About Opportunity and Change programme
We are funded to provide specialist Care Act 2014 assessment and after Care Act support work. We work in close partnership with: Acorn Training, Derventio and Women’s Work throughout Derby city and Derbyshire to ensure that the participants they support on this programme can access additional support at home before they are best placed to start their journey on becoming work ready.

Who works within the Care Act team?
We have a qualified & certified Social Worker and a trained Advocate who work with individuals that require 1-2-1 support to live independently within their own homes.
What Support will Social Worker and the Advocate Provide?
The Social Worker assesses individual’s eligibility for a Care Act assessment and if s/he is eligible, she will then carry out an assessnent of needs for a Personal Budget.
If s/he is not eligible for an assessment nor for a Personal Budget, then the Care Act team will do their best to ensure each participantl on the Opportunity and Change programme can access services provided by voluntary and/or public sector.
In partnership with Opportunity and Change’s delivery partners, each participant has access to:
- Assessment of needs including health & well-being
- Good housing
- Support to assist cooking, washing, dressing, communication, reading, managing finances and managing their mental health.
- Support with current financial circumstances.
This is with a view of enabling participants to move forward to become work ready.

- Carry out an assessment of needs including health & well-being,
- Check that the participant is living in appropriate & secure housing,
- Benefits check,
- Assist in accessing support to help with cooking, washing, dressing, communication, reading, managing finances and/or managing their mental health (where appropriate and meet eligibility requirements).
- Support with current financial circumstances.

- Cannot advise participants.
What to do Next
Who can we help?
We can refer individuals to one of the Opportunity and Change delivery partners but they will need to be eligible by meeting the below criteria:
- You are currently unemployed or economically inactive (e.g. a carer, looking after small children).
- You live in Derby or Derbyshire.
- You have the right to live and work in the UK.
